SUSD Health Services in partnership with San Joaquin Public Health Services is offering FREE FLU VACCINES to SUSD families and staff.  We are encouraging all children and adults to get vaccinated during the 2020 Flu Season.  Protect yourself and your family by getting a flu vaccine.

There will be drive-thru clinics here at Chavez High School on Tuesday, September 15 and Thursday September 17 from 11 am to 2:15 pm.  If you already submitted a flu permission slip, the form will be on file at the flu clinic event.  Additional forms can be found on the SUSD website: https://www.stocktonusd.net/cms/lib/CA01902791/Centricity/Domain/150/Annual%20Influenza%20Vaccine%20Consent%20Form%20English%20and%20Spanish.pdf. 

Siblings from other schools may receive a flu vaccine at Chavez High School with a signed parent consent.

Please wear a mask to the drive-thru flu clinic.  If possible, bring your own pen to sign forms.
